  1. Validity Period: This exclusive arrangement is valid from 1 November 2025 to 31 October 2026.
  2. Online Store Purchase Process: Customers may purchase the designated health check plan through the specified re:HEALTH online store and must enter the exclusive code at checkout to complete the payment. Upon confirmation of the transaction, a customer service representative will contact the customer to arrange an appointment. If the customer service team is unable to respond within the designated time, customers may contact us via WhatsApp at (852) 5726 4497 or by phone at (852) 2156 5857, and re:HEALTH will be pleased to follow up.
  3. Identity Verification: When visiting any re:HEALTH centre, customers must present a DBS Eminent Visa Signature Card or DBS Eminent Visa Platinum Card issued by DBS Bank (Hong Kong) Limited to access the designated services.
  4. Membership Registration: To protect customer interests and ensure service accuracy, customers are required to present a valid identification document and complete membership registration.
  5. Exclusive Use: The designated services are for the cardholder’s personal use only and are non-transferable.
  6. Service Confirmation: Prior to receiving any service, customers must ensure they fully understand and agree to the items and arrangements provided by re:HEALTH or its designated centres.
  7. Amendments: re:HEALTH reserves the right to revise, modify, or amend this exclusive arrangement and its related terms and conditions at any time without prior notice.
  8. Final Decision: In the event of any dispute regarding the related services, re:HEALTH reserves the right of final decision.
